Shaping Health and Wellbeing in Herefordshire
Scope
The scope of the review will be undertaken in line with the key aims and includes all current health and social care providers in a range of professions, including:
* Herefordshire Primary Care Trust provider services, including: general practices; community nursing; health visiting; physiotherapy; occupational therapy; podiatry; health psychology; phlebotomy; speech and language therapy; pharmacy; dentists; opticians; the PCT training centre.
* The services are provided in: people's own homes; general practices; health centres owned or leased by the primary care trust; other community services.
* Services provided to homeless people; people with learning difficulties.
* Community hospitals and care centres: Bromyard Community Hospital; the Hillside Centre; Kington Court Health and Social Care Centre; Ledbury Community Health and Care Centre; Leominster Community Hospital; Ross-on-Wye Community Hospital.
* Herefordshire Council social care services, including: adult social care; children's social care;
* Hereford Hospitals NHS Trust.
* West Midlands Ambulance Trust.
* Herefordshire voluntary sector providers.
* Private providers
