Shaping Health and Wellbeing in Herefordshire
Herefordshire Primary Care Trust has commissioned a review of how
health and social care services are managed and provided for the
benefit of local people, and in this review, is working alongside
Herefordshire Council, Hereford Hospital NHS Trust and local groups
and professionals.
The review is called Shaping Health and Wellbeing in Herefordshire.
A wide range of stakeholder groups and professionals are involved
in developing a set of principles and criteria and options designed
to improve health and social care for local people using local
organisations. This web site has been set up to keep everyone
informed about the aims, scope, context, process and progress of
this important review.
